Beneficiaries, who would like to email their primary care manager from a smartphone, tablet, laptop or desktop computer, can do so through a free patient portal.
Sign up for Naval Branch Health Clinic Albany’s free patient portal at www.RelayHealth.com. Use it to email the health care team, request non-urgent appointments, receive lab results, renew medications, access a health library and get health e-news.
“It saves time for routine questions like lab tests,” Dave Bragg, a retired chief petty officer and three-year RelayHealth user, said. “And signing up was easy. If I can do it, anyone can.”
RelayHealth is secure and offers 24/7 accessibility. Patients generally receive a response from their health care team within one business day.
“Our RelayHealth patient portal connects you to your health care team from any mobile device or computer — whenever and wherever you need,” Capt. Troy Borema, director for medical services and family medicine physician, Naval Hospital Jacksonville, Florida, (NBHC Albany’s parent command) said. “It can save a phone call or sometimes even a trip to the doctor.”
RelayHealth is one of the advantages of being enrolled with a PCM and Medical Home Port team at NBHC Albany. The team focuses on meeting the patient’s needs — for urgent, preventive and routine care. Each team has also earned highest-level recognition from the National Committee for Quality Assurance.
To reach the NBHC Albany Medical Home Port Olive Team, patients can use the RelayHealth patient portal or call the appointment line or Nurse Advice Line. There’s also a mobile app.
