July 1, 2015 -- Naval Branch Health Clinic Albany, aboard Marine Corps Logistics Base Albany, is conducting school physicals and immunizations — including a special clinic July 25, from 8 a.m. to noon, by appointment only.

For immunization questions, just stop by or call 229-639-7815. The Immunization Clinic is open Mondays through Fridays, from 7:30 a.m. to 4 p.m., and sees patients on a walk-in basis.
Georgia law requires health exam and immunizations documentation for all K-12 students. A school entry health exam (Georgia Department of Public Health Form 3300) is required for all school children who are entering a Georgia school for the first time (or after being absent from school for more than 30 days or one school year).
Students can obtain the exam up to 120 days prior to entering the Georgia school system. For new students transferring to a Georgia school, a comparable form from another state is acceptable, if completed within the previous year.
School physicals for children, ages 5-11, are covered by TRICARE Prime, if required in connection with school enrollment, unlike annual sport physicals, which are not a covered benefit.
Children, ages 5 and under, must first see their Medical Home Port primary care manager to ensure they get the appropriate immunizations. Parents should bring all immunization records to the clinic visit.
A certificate of immunization (Georgia Department of Public Health Form 3231) is also required for school entry and again before a student enters seventh grade, when new immunizations are needed. Students can get these additional immunizations while attending sixth grade, as soon as they’re age 11.
