March 20, 2014 -- Naval Branch Health Clinic Albany offers multiple ways for patients and their care team to connect.
Patients with a primary care manager at NBHC Albany can use RelayHealth, a secure e-mail service.
Patients can e-mail their care team and request lab results and medication renewals. Because RelayHealth is for non-urgent issues, it can take up to three business days for the team to reply to patient e-mails. The system is secure and confidential, with no cost.
To sign up for RelayHealth, visit the website, http://www.relayhealth.com/ or the Naval Hospital Jacksonville, Fla.’s, website, www.med.navy.mil/sites/NavalHospitalJax. Patients need to know the name of their PCM to sign up.
For technical help, call 866-RELAY-ME (866-735-2963). Patients can also sign up in-person at NBHC Albany.
The clinic care team is available by telephone during clinic hours, and nurse advice is available after-hours. Call 229-639-7884/7886, Mondays-Fridays from 7:30 a.m. to 4 p.m.
For after-hours nurse advice, call 800-529-4677 on evenings, weekends and federal holidays.
These communication options are a few of the features of Medical Home Port—the Navy’s approach to the nationwide “medical home” model of quality care.
Medical Home Port places the patient at the center of a collaborative team of caregivers—from doctors to nurses and case managers—led by the PCM.
The patient and team work together to meet the patient’s preventive, routine and urgent care needs.
“We’re strengthening the patient-provider relationship—in an environment where both staff and patients PCS and deploy—with team-based care,” Capt. Troy Borema, a family medicine physician, NH Jacksonville, Fla., said. “Patients’ secure e-mail access to their team is one aspect of that care.”
